Maintains an accurate, efficient and timely electronic health record keeping system of KANA patient charts and clinical documentation. Ensures all patient information release is within the guidelines and regulations set forth by HIPAA and AHIMA, and KANA policy and procedure.

  • Support the organization’s mission and goals, quality standards, and patient-centered medical home philosophy.   • Uphold KANA’s Code of Ethics by conducting professional activities with honesty, integrity, respect, fairness, and good faith in a manner that reflects positively upon the organization.
  • Electronically file medical documentation in the individual patient medical charts via scanning/imaging software.
  • Performs quality control checks to monitor for correct patient name, account number, medical record number, and document titles.
  • Verify a copy does not already exist in the patient’s chart prior to scanning a duplicate copy. Ensure all records are filed chronologically and titled efficiently so that documents are easily retrieved by the end user.
  • Protect the security of medical records to ensure confidentiality is maintained. Strictly adhere to the provisions of the Privacy Act, HIPAA and KANA policies. Verify release of information (ROI) paperwork are completed properly and releases medical records to patients and 3rd party requestors with proper authorization.
  • Archive records when the patient’s status becomes inactive according to the NARA record retention guidelines and established protocols.
  • Provide cross-coverage for other positions within the Patient Access Department, as assigned.
  • Communicate effectively and appropriately with patients, colleagues, and other KANA staff. Practice meaningful customer service skills addressing patient and staff needs courteously, promptly, and with compassion.

Requirements

High school diploma or general education degree (GED) and one year related experience and/or training; or equivalent combination of education and experience.
