This position will be in located Ouzinkie.

Candidate must live in Ouzinkie or be willing to relocate to Ouzinkie to be eligible for this role. 

Provides direct advocacy and support to victims of crime such as sexual assault and/or domestic violence. Performs multiple roles under this pilot program; serve as village point person, victim advocate, and assists victims in navigating complex criminal justice and social service systems. Responsible for collaborating with local VPSO, the local Tribe, local municipal government, and community partners on service delivery.

  • Provides advocacy services and intensive case management to survivors of relationship violence, stalking, and sexual assault or abuse in coordination with other community agencies.
  • Develops and establishes safety plans; monitors victim circumstances that may impact victim’s safety.
  • Develops recovery and self-sufficiency plans with clients, guiding clients through referrals and coordinating case management through setting goals and taking necessary steps to achieve goals, and apply knowledge of professional social work principles and methods to help clients recover and achieve self-sufficiency.
  • Administers initial and ongoing assessments to identify each victim’s accomplishments, strengths, and current or new service priorities in need of support.
  • Assists and refers clients, as necessary, during crisis and work toward stabilization; assist clients in the development of immediate, short-term, and long-term goals; provide service coordination and aid in navigation of agencies.
  • Attends and participates in all relevant and regular case coordination meetings and conference reviews, organize case meetings when needed to review case progress, client strengths, barriers, and needs as well as impediments to safety or self- sufficiency.
  • Assists and facilitates services among clients, including legal and social service agencies necessary for client recovery and self-sufficiency, and maintain compliance in various assistance programs. Creates timelines and task lists, verifies applicable program compliance requirements, identifies priorities, and facilitates communications between programs and clients.
  • Determines client needs, identifies resources to fill needs, facilitates client connection to resource provider, monitors client interactions with providers, and provides supportive services as needed to improve outcomes or remove barriers.
  • Guides clients in recovery and goal setting, and assists them with identifying personal strengths. Provides coaching to clients on resolving problems that may arise during recovery and self-sufficiency. Provides clients with short-term crisis intervention and stabilization, and support clients with establishing long-term self-sufficiency plans.
  • Maintains current casefiles and reports on each client assigned, including agency and grant-required information.
  • Identifies gaps in service delivery and develop corrective action plans as needed, or make referrals for and coordinate with wrap-around services. Provides ongoing monitoring of clients for basic needs assistance.
  • Maintains contact with agency partners through releases of information (ROIs) from clients. Verifies participation in other programs necessary for client recovery and self-sufficiency, such as behavioral health, substance use, housing, community services, among other resources. Provides intensive case management support to village clients in the form of regular face-to-face meetings, phone conferences, or video conferences.
  • Responsible for programmatic operations of grant funded pilot program: receive and coordinate client referrals, including eligibility determination; day-to-day case management; maintenance of client case files in accordance with grant requirements; case closure; facilitates local victim support group, and other requirements as determined.
  • Understands and follows program grant plans, policies and procedures. Assists Family Services Coordinator with modifying grant plans, policies, and procedures as necessary.
  • Remains current on available resources in the village designated under the pilot program, as well as statewide and national resources available for victims of crime.
  • Attends relevant and mandatory trainings and meetings in order to more effectively adhere to grant requirements, keeps abreast of grant policies, procedures and law and regulation changes, and gains knowledge in ways to improve the pilot program, current advocacy and victim service programs.

Requirements

High school diploma or equivalent and two years related experience in social services, human services, or community relations work involving client interaction, referral services, and case management; or equivalent combination of education and experience. Associate’s or bachelor’s degree in Social Services or related field preferred.
