Provides comprehensive case management services for substance use treatment clients needing ancillary supports that will improve their quality of life, plays a key role in administration of the Contingency Management program, manages processes related to GPRA data collection, administers substance use tests as appropriate, and supports clients who are engaged in substance use-related services.
Essential Duties and Responsibilities Under the direct supervision of the Substance Use Program Supervisor, job incumbent may be assigned from the following representative duties:

- Conduct brief case management assessments to determine client needs.
- Provides client-centered, intensive case management which includes active support to minimize barriers that prevent clients from reaching treatment goals. Provides resources for addressing areas of financial, social, educational, legal, and broader treatment needs.
- Provides coordination of care with referral sources and partners, both within and outside of KANA. Coordinates transfer to and from inpatient care, including medical appointments, agency applications, and applications for alternate resources. Facilitates warm hand-offs and smooth transitions between agencies to ensure high-quality continuity of care.
- Establish and maintain strong relationships with referral agencies, including residential treatment centers, hospitals, Alcohol Safety Action Program (ASAP), Adult Probation, Kodiak Teen Court, the Office of Children’s Services, and other relevant community agencies.
- Maintain updated information regarding the bed availability of both in- and out-of-state residential treatment programs to provide individualized and timely coordination of care.
- Provides crisis stabilization for current substance use disorder (SUD) clients as appropriate.
- Receives direction from relevant Behavioral Health supervisors/managers regarding clinic processes, programming, and expectations.
- Assists clinical staff with correspondence and records requests between community and compliance agencies and KANA.
- Assists in the facilitation of outpatient and pre-treatment groups as appropriate. Provides psychoeducation, motivation to change interventions, local resource education and connection, and other treatment needs as needed.
- Participates in clinical meetings case reviews to provide updates regarding client progress towards treatment goals; assists the team in determining further interventions as needed.
- Completes administrative and clinical documentation in a timely manner in order to maintain program compliance within the guidelines established by Kodiak Area Native Association, State of Alaska, Medicaid and other regulatory agencies. Compiles data and reports on request.
- Works with providers to coordinate Individual Service Agreement (ISA) purchases on behalf of clients.
- Coordinates and tracks all aspects of Government Performance and Results Act (GPRA) data collection to include: screening clients for eligibility, administering client surveys/interviews, documenting responses, entering data into the appropriate electronic platform, and tracking clients through multiple touch points. May coordinate other types of surveys, such as discharge surveys, on behalf of the department.
- Attends required training and meetings; works toward becoming a Qualified Addictions Professional as part of professional development plan.
- Communicates regularly with other members of the client’s care team, including referral sources, medical providers, administrative support staff, etc.
- Administers substance use tests as appropriate and applicable. Provides clients with education surrounding substance use testing, substance interactions, and other topics as relevant. Coordinates reports back to relevant treatment providers concerning clients, as well as reports client compliance to external legal agencies as required.
- Support relevant staff in the implementation and ongoing development of a Contingency Management (CM) program for eligible SUD clients. Coordinate distribution of CM vouchers to clients, purchase and distribution of CM incentives, and comply with all applicable guidelines and policies/procedures related to CM program documentation.

Requirements
Associate’s degree in related field and 2 years of relevant work experience; or any equivalent combination of education and experience.
